OLBS Religious Education

In the event that the above-signed, or my authorized physician, cannot be reached and in the judgement of a responsible OLBS staff member, there is a necessity for immediate examination and/or treatment of my child/children enrolled in OLBS Religious Education, I hereby request and authorize any of the aforementioned personnel to obtain for my (our) child/children such medical services as are deemed necessary. I agree to assume the financial responsibility for any such diagnostic/medical treatment and/or for any medication deemed necessary. Printing your name above where indicated will be considered your signature and authorization. Please include parental affiliation (Mother/Father/Guardian).

I hereby authorize OLBS/OLBS Religious Education to photograph and/or record my child/children during educational or religious activities for the purpose of praising and encouraging their participation and spirit as demonstrated in their religious education classes, activities and other faith formation experiences as well as for advertising and promotional purposes for Our Lady of the Blessed Sacrament ("Parish") and/or Our Lady of the Blessed Sacrament Religious Education ("RE"). I understand and agree that any such photographs or recordings of my child/children may be posted to Bulletin Boards in the Parish and/or School, Weekly Parish Bulletin, Parish Website or Parish/School social media channels including but not limited to Vimeo, YouTube, Facebook. By printing your name where indicated above, you authorize use of the likeness of children in your care. Please include parental affiliation (Mother/Father/Guardian).
