PAYMENT (READ FIRST)

  • You must pay in full the tuition and/or sacrament fee at the time of registration. Payment must clear and be verified before being deemed paid in full to complete enrollment.
  • If you are not able to pay the tuition in full today, do not proceed with trying to register your child through GiveCentral. Contact the office at 410-676-5119, to make an appointment to meet with Chris Brooks, ext 121 or Diane Lewis, ext. 115. Payments will need to be made through GiveCentral if approved.

FAITH FORMATION TUITION FEES

  • One Child: $80; You must enter 1 in the ticket. You will be charged $80 for one child.
  • For two or more children you must enter 1 ticket to be charged the discounted fee of $135. This fee will cover two or more children you enrolled.

SACRAMENT FEES (This is an additional fee to the tuition. You must enter the total number of sacraments in tickets for all children enrolled that require sacraments. For example, if you have three children enrolled, 2 of the three children need their First Eucharist sacrament so you would enter 2 in tickets which would be $140 due. This amount should be reflected in the box next to First Eucharist tickets.)

  • NO SACRAMENT $0

  • LIABILITY WAIVER

    I, as the parent or legal guardian of the student(s) named on this form, hereby provide permission for my child(ren) to participate in the faith formation program at my own initiative and application, and I accept any associated risks. I hereby release The Pastorate of Prince of Peace, Edgewood and St. Francis de Sales, Abingdon, and the Archbishop of Baltimore, a corporation sole, along with its Board Officers, employees, and volunteers, from any claims of liability arising from any accident or injury to my child(ren) that may occur during or as a result of my child's participation in the Faith Formation Program, in exchange for his or her participation in said program.
