PAYMENT (READ FIRST)

  • You must pay in full the tuition and/or sacrament fee at the time of registration. Payment must clear and be verified before being deemed paid in full to complete enrollment.
  • Do not proceed registering your child through GiveCentral if unable to pay in full. Contact the office at 410-676-5119, to make an appointment to meet with Fr. John. 

FAITH FORMATION TUITION FEES

  • One Child: $80; You must enter 1 in the ticket. You will be charged $80 for one child.
  • For two or more children you must enter 1 ticket to be charged the discounted fee of $135. This fee will cover two or more children you enrolled.

SACRAMENT FEES This fee is in addition to the tuition. You must enter the total number of sacraments in tickets for all who are enrolled. For example, if you have three children enrolled, two of them need their First Eucharist/Reconciliation sacraments; therefore, you would enter two tickets, totaling $200. Record this sum in the box next to the First Eucharist/Reconciliation tickets.

  • First Eucharist/Reconciliation $100
  • NO SACRAMENT $0

FAITH FORMATION TUITION & SACRAMENT FEES

  • LIABILITY WAIVER

    I, as the parent or legal guardian of the student(s) named on this form, hereby provide permission for my child(ren) to participate in the faith formation program at my own initiative and application, and I accept any associated risks. I hereby release The Pastorate of Prince of Peace, Edgewood and St. Francis de Sales, Abingdon, and the Archbishop of Baltimore, a corporation sole, along with its Board Officers, employees, and volunteers, from any claims of liability arising from any accident or injury to my child(ren) that may occur during or as a result of my child's participation in the Faith Formation Program, in exchange for his or her participation in said program.

    I have read, understood, and agree with the liability waiver. I willfully sign the waiver to enroll my child(ren) in the program. I am also aware that there may be a delay in my child(ren)'s class enrollment if I do not sign this agreement.
